This sheet is a risk assessment to show our cast and crew all the potential hazards at the different locations. It has also has how I will try and prevent the hazards and stop people from getting hurt. This sheet also includes the names of the crew who specialize in health and safety.

This sheet is to show that we have permission to film in the specific location and it shows the what days and what times we can film in the location. It also has the land owners name, email and phone number on incase we need to contact them about any concerns.

I have filled in this location sheet form to show my cast and crew where we will be filming and how long it will take to film the scene. This form is just to inform people of the location address and what dates and times we will be filming there.
